The Case Manager is part of a team supporting individuals who are experiencing homelessness, severe mental health conditions, and substance use disorders in Davis, California at the Davis Respite Center. The community health center movement was born out of the civil rights movement in the 1960s with the mission to provide healthcare in communities where there are few or no options for care. About the role of the Case ManagerResponsible for providing on-site case management, crisis support, care coordination, service linkage and services for those persons seeking support at the Davis Respite Center.The Case Manager will be able to make contact, engage and accurately attune to the needs and goals of each client and deliver all aspects of service with cultural humility and responsiveness, acceptance, and respect. The Case Manager will have an understanding of harm reduction principles and the social determinates of health and be able to respond to a variety of presenting concerns including substance use, severe and persistent mental illness, trauma, and physical health conditions. The Case Manager consults regularly with Yolo County Health and Human Services, City of Davis, other members of CommuniCare Health Centers staff, and community partners. The Case Manager will link clients with vital resources and support them to successfully engage in services. The Case Manager provides collaborative and strength-based interventions that support stability and promotes independence.Essential FunctionsThe following reflects managements definition of essential functions for this job but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or other reasons.Actively support and encourage individuals experiencing homelessness by continual efforts to be responsive, flexible, patient, and trauma responsive. Advocate for a culture of a low threshold access to services and work to reduce barriers to stability.Serve as a primary in-person service coordinator and actively consults with managers and supervisors when situations arise.Complete intakes and needs assessment with clients, orient to services and safety, and support each person to understand Respite Center policies.Utilize knowledge of harm reduction principles, verbal de-escalation techniques, motivational interviewing and will consistently use non-stigmatizing language and model a non-judgmental approach.Collaborate effectively with internal and external partners in order to support the goals of each client.Utilize and collect required screenings, assessment, and data collection tools as required by agency or contract. Presents options to clients with cultural humility and respect for personal choice.Demonstrate competence with the technology required to perform job duties including Language Line, Microsoft Office, eClinicalWorks and video-based telehealth and meeting platforms utilized by the agency. Complete documentation within 48-hours of services in accordance with working hours, all documentation must comply with Medi-Cal, Partnership, Beacon Health, and/or other contractual guidelines.Learn and understand billing structure; correctly utilize service codes. Respond to calls, emails, and other correspondence from internal and external partners in a timely and professional manner. Monitor charts to ensure the timely placement of all correspondence, completes all screening and data collection tools according to protocol and best practices. Supports with data collection and reporting.Maintain confidentiality and the privacy of residents and their information unless permission is provided or situation warrants disclosure (e.g. safety)Assist with other projects as assigned.As applicable, maintains current license / registration in good standing, submits all CE and training certifications to HR. If CPR certification is required, will obtain within 30 days of hire.Comply with all legal, ethical, safety, and agency expectations that govern the delivery of case management services.Education & Experience: Bachelors Degree in related field preferred or experience in lieu of degree. Experience working in a mental health, social service, or field-based/community setting required. History of working with people experiencing substance use disorders and/or homelessness and chronic poverty strongly preferred. Experience providing services in culturally responsive and trauma-informed manner. Crisis management skills a plus.Special Skills/Equipment: Bilingual preferred, Spanish/English strongly preferred. Computer skills to include Microsoft Office (Word, Excel, Outlook). Must provide current driver license and auto insurance, ability to travel in Yolo County (West Sacramento, Davis, Woodland).Expectations: Commitment to CommuniCares mission and values will be made evident in the delivery of services and all professional interactions. Attendance and punctuality are vital. Ability to collaborate effectively with a multidisciplinary team including community partners. Must have excellent communication (verbal, written), organizational and problem-solving abilities. Required to have versatility, flexibility and manage multiple priorities with enthusiasm and attention to detail. Expected to have the ability to understand and follow written/verbal instructions. Applicant must be able to able to provide services outdoors and at homeless encampments. Able to work independently and set priorities effectively. Required to have the ability to serve a diversity of patients in ways that attend to strengths and resilience with cultural humility and respect. Effectively navigate fast-paced workday. COMPLETED COVID VACCINATION REQUIRED.
