Job Description:

The Registered Dietitian Nutritionist (RDN) is a valued member of the Food and Wellness team and serves as the nutrition lead for Healthy Living with Diabetes and Food is Medicine programming. The RDN will facilitate individual medical nutrition therapy and group education activities. RESPONSIBILITIES:The following reflects management's definition of essential functions for this job, but does not restrict the tasks that may be assigned.   • Working with Primary Care clinical and program staff, provide evidence-based, patient-centered medical nutrition therapy to patients encountering diet-related disease
  • Accurately document interactions and follow up to support patient's health journeys.
  • Develop and implement healthy lifestyle classes for pediatric patients at risk of diet-related disease and their family members.
  • Serve as a nutrition resource for Food is Medicine and Healthy Living with Diabetes activities, including culinary education, produce prescriptions, and nutrition education for patients with diabetes.
  • Working with program staff, ensure all outcomes are reached and reporting complete as outlined in the Healthy Living with Diabetes and Culinary Education grants.POSITION REQUIREMENTS:Education & Experience:
    • Registered Dietitian Nutritionist with the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics
    • Bachelor's or advanced degree from an accredited institution with a major in Nutrition, Dietetics, or related field
    • Experience providing individual nutritional counseling and facilitating group education sessions
    • Experience engaging children, adolescents and their caregivers in nutrition counseling and group education
    • Experience working with persons living with or at risk for diabetes and other diet-related chronic diseases
    • Demonstrated commitment to providing quality care to diverse, low-income, under/uninsured population
    • Demonstrated commitment to improving healthy food access and to advancing food justice and health equity Preferred Skills:
      • Bilingual (Spanish and English) written and spoken language skills highly preferred; other desirable language skills in addition to English include Russian and Punjabi
      • Ability to communicate effectively with patients and their family members.
      • Experience working in electronic health records.
