Overview:About us: University Retirement Community is looking for a passionate Resident Care Manager (RN Case Manager) to join our 5-star rated SNF! Our Skilled Nursing Facility cares for 37 residents and is located within our expansive Continuing Care Retirement Community in Davis, California. We provide compassionate Skilled Nursing and Rehabilitation for individuals who need short term rehabilitation or long-term healthcare assistance. To learn more about us, click here!What youll be doing: In this role you will be supporting our nursing staff, and collaborating with physicians, the interdisciplinary team, residents, and their families. Daily this can look like completing nursing assessments and tasks, advocating for patients, performing patient education, and covering the floor when staffing requires. Duties will also include assisting with admissions and discharges, attending care conferences, creating/updating care-plans, among other duties.Schedule: 11:00 am - 7:30 pm; on 3/off 3Wages: $45.00 - $55.00 / hr DOEAbout you: we are looking for driven and compassionate clinicians to join our team with the goal of providing exceptional care to our residents.
