Home Health Professionals for Home Health Speech Therapists (ST)

life. As home health nurses, you have an opportunity to teach and
educate your patients, apply your skills for treatments, and carry
doctors orders based on individual patient's needs. Job
Description: We are currently seeking dedicated healthcare
professionals to join our team. As a member of Mare Island Home
Health, you will have the opportunity to make a profound difference

comprehensive care tailored to their unique needs. Positions
available include: Registered Nurse (RN) Licensed Vocational Nurse
(LVN) Home Health Aide (HHA) Physical Therapist (PT) Physical
Therapist Assistant (PTA) Occupational Therapist (OT) Certified
Occupational Therapy Assistant (COTA) Speech Therapist (ST) Medical
Social Worker (MSW) Responsibilities: Conduct thorough assessments
and develop individualized care plans. Provide skilled nursing
care, therapy services, or social work support according to
prescribed treatment plans Educate patients and caregivers on
disease prevention and management. Collaborate with
interdisciplinary team members to ensure holistic patient care.
Document all patient care services accurately and timely. Flexible
time for home visit One on one with your patient Independent use of
professional skills Continues education and professional growth
Initial Requirements: Valid California state licensure or
certification in your respective field (RN, LVN, PT, PTA, OT, COTA,
ST, MSW). Current CPR certification Current CA driver's license
plus proof of car insurance. Minimum one year ST clinical
experience, (two years is more preferable) Computer/Internet
experience (online charting to be used) Bi/Multi-lingual skills a
plus (e.g. Spanish, Russian, Tagalog, etc.) Ability to travel to
patient homes across Solano County, Napa County, and Contra Costa
